Community Health Workers (CHW) will work closely with medical and dental providers, primary care teams, and other agencies to improve patient care and outcomes. CHWs help to identify barriers to care and work to address and remove these barriers, when possible.
CHWs will be involved with all areas of clinic operations, including scheduling appointments, assisting with insurance applications, and helping to coordinate care between departments and clinic staff. CHWs will focus on utilizing face-to-face encounters with patients to determine how best to support patients in their healthcare efforts and social determinants of health needs.
One of the Community Health Worker Positions will primarily focus on the Dental Team, the patients with Dental Care and our Dental School Program. For this CHW Position previous experience working in a dental clinic is helpful.
Community Health Worker Position with a primary focus working with children and the needs of children. For this CHW Position previous experience working with children from birth to 18 years of age, working with children with special needs.
Community Health Worker Position in our California Clinic working with patients in need of resources, must be bilingual for this position.
Bachelor’s Degree in a social-services or public health field is ideal for this position but will consider other related degrees and experience. Bilingual a real plus for these positions.
Certification as a Community Health Worker is preferred but certification will be offered by the Community Health Center if not already obtained prior to employment at no cost to the staff member.
