The ideal candidate should be experienced and demonstrate commitment to working with those with mental illness, substance use disorders and/or chronic health conditions. Job duties include and are not limited to identifying and determining resources in the community, coordinating, and linking clients to providers, actively intervene and advocate on behalf of clients as needed, provide follow up and transitional services, maintain program documentation as required and adhere to appropriate Medicaid billing requirements.
Qualifications:
Qualifying fields of degree: social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services.
- Master’s Degree in qualifying field with one year experience or a Bachelor’s degree in any field with 3 years’ experience.
or
- Credentialed CASAC or CASAC-T with bachelor’s in Mental health Counseling, education, nursing, psychology or social work AND three years’ experience providing direct services to persons with mental illness and/or substance abuse.
