Willow Springs Care and Rehabilitation Center has a full-time and/or a part-time opening for an OT to work alongside our experienced staff of in-house therapists to deliver outstanding rehab care in our SNF. Opportunities exist to deliver inpatient care to our short-term patients as they recover before returning home and to our long-term residents to improve their function / quality of life.

Essential Job Duties
- Provides Occupational Therapy services to residents and initiates referrals when appropriate and in accordance and in accordance with Occupational Therapy Department Procedures.
- Conducts screenings of residents at regular intervals to determine need for intervention/treatment.
- Evaluates residents to obtain data necessary for treatment planning and implementation and conducts specialized evaluations as indicated.
- Interprets and communicates findings to residents, families and team members
- Establishes individual resident schedules in conjunction with other team members.
- Develops treatment plans by identifying a target list, including short- term and long-term goals and methods to achieve identified goals.
- Coordinates treatment plan with resident, family and other team members.
- Implements and conducts treatment as outlined in treatment plan.
- Monitors resident’s response to intervention and modifies treatment as indicated to attain goals in accordance thin physician orders.
- Develops appropriate home or programming within the community to maintain and enhance the performance of the resident in their own environment.
- Documents results of resident’s assessment, treatment, follow-up and termination of services.
- Adapts treatment to meet the individual need of the resident.
- Provides, recommends and fabricates adaptive devices or other equipment. Trains resident, family and appropriate staff in the use or application of such equipment.
- Participates in discharge planning.
- Supervises certifies occupational Therapy Assistants (COTA) in the implementation of the treatment plan.
- Reviews the quality and appropriateness of the total services delivered and individual occupational therapy programs for effectiveness and efficiency, using predetermined criteria
- Participates in resident, family and staff education.
- Confers formally and informally with other team members in coordinating the total rehabilitation program of the resident.
- Adheres to Occupational Therapy Department procedures regarding documentation of occupational services.
- Provides in-service education to the members of the resident's care team and education to the family.
- Provides treatment in accordance with established standards of practice, department procedures and productivity standards.
- Consistently demonstrates sound judgment in the evaluation, planning, implementation and follow-up of resident therapy programs.
- Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift.
- Assists with and participates in Quality Assurance Programs as required
- Maintains a current knowledge of the law and regulations regarding rehabilitative services and consults with the supervisor to ensure that all employment actions are in compliance with applicable applications.
- Attends and participates in scheduled trainings, educational classes and meetings required for requested for position.
- Ensures a safe environment and utilizes tools and equipment in a safe manner complying with all infection control, universal precautions, and OSHA standards for the healthcare personnel.
- Reports all hazardous conditions including incidents, injuries and equipment to Supervisor/ appropriate personnel immediately.
- Responds to and acts appropriately in emergency or disaster situations.
- Adheres to high standards of cleanliness, grooming, hygiene and dress code.
- Adheres to all policies of company including job description, certification requirements, Mission Statement, HIPPA, confidentiality standards and resident's rights.
- Adapts to changes in responsibility in a positive manner, is receptive to new ways of completing tasks and is flexible in carrying out assignments.
- Communicates and interacts professionally and effectively with residents and coworkers throughout the community ensuring a cooperative environment while maintaining self-control
- Communicates and interacts professionally and effectively with family members in attending to and meeting their requests.
- Performs all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the organization.
Basic Qualification
Education/Training:
- Bachelor of Science Degree in Occupational Therapy from an accredited program. Successful completion of National Certification Examination for OTR. Assumes responsibility for ongoing continuing education and professional development.
Skills:
- Moderate reading, writing, grammar and mathematics skills; proficient in interpersonal relations and communicative skills; auditory and visual skills, ability to bend, stoop, sit, stand, reach, and lift items weighing 50 pound or less.
- Current state licensure and/or certification/registry as required.
