Summary: The Resource Coordinator plays a crucial role in facilitating clients' access to service entitlements and delivering support services aligned with the Psychosocial Rehabilitation Model and Strengths Model. Tailored for an adult population facing challenges related to mental health, substance use, and justice involvement, the Resource Coordinator provides recovery-oriented services, including assessments, case management, skills training, and crisis intervention. Operating in various environments such as clients' homes, correctional facilities, psychiatric hospitals, or agencies, the specialist is responsible for identifying and establishing connections with community resources and partners.
Essential Duties and Responsibilities:
- Client Transition and Community Engagement:
- Assist with client intakes, coordinating with discharging facilities and other community agencies.
- Conduct field visits, including transporting clients to and from services.
- Facilitate linkage of clients to community services, attending meetings and networking as needed.
- Support the transition of clients to outside facilities to ensure continuity of care.
- Client Assessment:
- Conduct comprehensive assessments, focusing on identifying needs related to benefits and housing support.
- Collaborate with the treatment team and support persons to gather relevant information regarding clients' mental health and social needs.
- Benefits Coordination:
- Support clients in applying for and obtaining relevant benefits, including General Assistance, Medicaid, Social Security Disability, and other financial assistance programs.
- Stay current on changes to benefit programs and policies, providing accurate and up-to-date information to clients.
- Housing Support:
- Collaborate to develop comprehensive housing plans, considering safety, accessibility, and individual client requirements.
- Assist clients in navigating housing applications, subsidies, and support services for secure and stable housing.
- Identify appropriate housing resources, evaluating potential options to meet livability standards and align with clients' well-being.
- Conduct regular check-ins on clients' living arrangements to address emerging issues promptly.
- Advocacy:
- Advocate on behalf of clients to ensure fair and just treatment in securing benefits and housing.
- Collaborate with external agencies, social services, and community organizations to enhance available resources.
- Empower clients with information on self-advocacy, budgeting, and essential skills for stable housing and benefit access.
- Documentation and Reporting:
- Maintain accurate and timely documentation of client interactions within the Electronic Health Record system.
- Generate reports on the status of client benefit and housing applications, providing regular updates to the clinical team.
- Collaboration:
- Work independently and with a multi-disciplinary team to discuss, develop, and facilitate integrated care plans.
- Contribute insights to ensure comprehensive and integrated support for clients.
- Facilitate linkages to community resources that enhance individual well-being.
- Professional Development:
- Actively engage in training sessions, workshops, and conferences to enhance skills and knowledge.
- Adhere to and uphold El Hogar Community Services Inc.'s policies and procedures.
- Ensure the protection and privacy of all Protected Health Information (PHI) following agency policies.
Supervisory Responsibilities: This position does not involve any supervisory responsibilities.

Education and/or Experience: Minimum qualifications can be met in the following ways:
- High school diploma or equivalent (required)
- Experience providing housing services to an under-served population (Preferred)
- Familiarity with fair housing laws and regulations, HMIS, SOAR (Preferred)
- Knowledge of resources for people experiencing homelessness in the Sacramento area (Preferred)
- Direct care experience in a mental health related field providing services to individuals with a Co-Occurring diagnosis and/or involved in the justice system (Preferred).
