The Pharmacist contributes to the quality and effective operation of the department by ensuring safe, appropriate and cost effective medication therapies for patients according to established policies, procedures and protocols. Oversees all department activities pertaining to the preparation and dispensing of pharmaceutical preparations, in accordance with federal, state and local laws and in compliance with the requirements of the Virginia Board of Pharmacy.
Duties and Responsibilities
- Interprets medication orders and transcribes to computerized patient medication profiles accurately.,
- Weighs, measures and prepares medication products by utilizing appropriate tools and equipment.
- Maintains accurate and complete patient medication profiles as evidenced by a lack of error reports.
- Provides clinical consultation and clarification to practitioners and patients and suggests appropriate cost effective therapeutic alternatives to medical staff as needed.,
- Ensures Pharmacy services are provided in a timely manner as evidenced by lack of complaints.
- Issues controlled substances to patient care areas and maintains records as required by law.
- Accurately compounds and dispenses pharmaceuticals including sterile, chemotherapy and parenteral nutrition products as evidenced by compounding logs.
- Orders items that fall below par levels or special ordered items, identifies and replaces outdated and unusable drugs as documented on unit inspection forms and documents inspections of all assigned medication storage areas.
- Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose documented by intervention reports.
- Detects and reports suspected adverse drug reactions accurately and in a timely manner by documentation of Adverse Drug Reaction reports.
- Sustains the formulary by minimizing non formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection by processing non-formulary drug requests and documentation in clinical intervention logs.
- Supervises and directs pharmacy support personnel, verifying their daily activities and participating in the performance appraisal process.
- Participates in department quality programs as evidenced by documentation of meeting attendance and reports produced.
- Takes ownership for daily department operations in the absence of the manager or supervisor in accordance with established procedures.
- May perform additional duties as assigned.
