This Position is IN PERSON Monday through Friday; 8am- 5pm
Purpose; The CSW provides direct care services to clients who qualify for Comprehensive Community Support Services (CSSS).
Comprehensive Community Support Services(CCSS) certification through the State are required within 90 days of hire. Krossroads will provide travel expenses to ABQ for the training, if needed. CCSS/case management is the backbone of our treatment model for the Krossroads Integrative Health and Recovery Solutions behavioral health and substance abuse clients. In this position you will be working with a multidisciplinary team of licensed professionals to provide assistance and support to help rebuild social foundations with clients. Certified Peer Support Workers (CPSW) and person in recovery (minimum 3 years) are encouraged to apply.
Duties
Knowledge, Skills and Abilities:
- Implements agencies program for “Full Circle Continuity of Care and Parallel Service Models”, policies and procedures
- Must be able to follow the “Comprehensive Community Recovery and Re-integration (CR2)” program for legal referrals
- Implement the CR2 Justice Involved Services (JIS) for all legal referrals
- Coordinate with the JIS Program Manager for all CR2 services provided to clients and successfully submit compliance reporting to legal program manager in a timely manner
- Provides Intake, Screening and coordinates with medical staff for assessments
- Manage caseload appointments to reduce or eliminate, "no-shows"
- The CSW interacts professionally with agency staff, community partners, and community resources as necessary.
- Build a collaborative relationship with upper management
- Provide 30 hours of billable services a week
- Provide all case notes within 24 hours of appointment
- Collaborate with administrative team to schedule clients for all services
- Provide direct services to clients, including but not limited to: skill development, building on strengths, identifying natural supports, modeling positive social skills, increasing health and well-being, and any additional support as needed for CSWs and client case load.
- Ensures all CCSS services provided are 60% community-based at minimum
- Assist client to establish positive relationships in the community
- Provide regular home visits to clients on caseload; assist team with home visits as necessary
- Ensures client and staff safety
- Supports client management of appointments and community engagement
- Follows agency incident reporting protocol
- Coordinate wrap around services in the community
- Refer clients to resources
- Coach clients to learn daily living skills thereby supporting a least restrictive care setting
- Provide, education, and support to maintain medication and other treatment regimens to both clients and staff (e.g. a CSW does not handle medication directly).
- May lead educational/skills groups for clients in areas such as basic living skills
- Utilizes Vocational Rehabilitation to support client’s employment goals
- Empowers each client to make his/her own life decisions
- Complete needs assessment, and updates as required
- Complete and updates CCSS treatment plan as needed, or within no less than a 90-day timeframe
- Assists clients to formulate and achieve goals
- Participates in treatment team and clinical meetings
- Provides family education and support
- Complete additional documentation as needed including but not limited to crisis plan, advanced directive, and relapse prevention plan, compliance plan for CR2 program clients
- Maintains healthy boundaries with client, and others that may be involved in their lives
- Is physically able to complete job duties
- Proficiency at digital literacy including Microsoft Word, e-mail, telemedicine, and EHR (electronic health record) and guide team in these areas as needed
- Uses Seeking Safety curriculum as a tool to support client empowerment, safety, and insight to increase progress towards goals as applicable
Qualifications
- Must be 18 years of age or older.
- Have a high school diploma or equivalent
- Previous history working in behavioral health and substance abuse
- Excellent communication skills verbally and written
