General Summary of Position
Directs, supervises and provides holistic, culturally competent, quality and cost-effective evidence-based nursing care for patients/families. Applies the nursing process within the context of the organization's shared decision making frame work. Collaboratively manages the patient's transitions across the continuum of care, to assist patients or populations in achieving or maintaining an optimal level of health. Is recognized as a professional role model and clinical expert and promotes a professional environment that supports continuous learning. Delegates and supervises work appropriately. The Patient Care Manager supports, assists and reports to the Nurse Director. Can assume the role of Nurse Director in his/her absence.
M*inimum Qualifications*
Education
- Bachelor's degree in Nursing required
- Master's degree or enrolled in a Master's program preferred
Experience
- 1-2 years of progressively more responsible job-related experience. required
Licenses and Certifications
- RN - Registered Nurse - State Licensure and/or Compact State Licensure in the District of Columbia required
- BLS - Basic Life Support required
- Health Services\ACLS within 180 Days required
- Certification in the clinical/specialty area within 1 Year required
- C-EFM - Electronic Fetal Monitoring from the National Certification Corporation (NCC) is required for Perinatal nurses (labor and delivery, antepartum, antenatal testing) within 12 months from date of hire. New graduate nurses or new to specialty nurses (labor and delivery, antepartum, antenatal testing) will have 15 months from date of hire to obtain the EFM certification.
Primary Duties and Responsibilities
- COMMUNICATION & RELATIONSHIP BUILDING: Supports a patient first culture that fosters a high level of patient experience as demonstrated by outcomes data.
- COMMUNICATION & RELATIONSHIP BUILDING: Collaborates with Nurse Director to hire, evaluate promote, initiate coaching, counseling and termination of departmental personnel as needed.
- COMMUNICATION & RELATIONSHIP BUILDING: Establishes, develops, and maintains effective relationships, working agreements and quality partnerships across the continuum to ensure quality patient care; to secure resources to support evidence-based patient care; and to assure compliance with current professional policies, procedures, and protocols and nursing standards. the Nurse Director, the Assistant Nursing Director maintains communication with the Senior Nurse Director, Associates and Medical Director or their designee for the purpose of coordinating operations between the department and the clinical division.
- COMMUNICATION & RELATIONSHIP BUILDING: In the absence of the Nurse Director, the Patient Care Manager maintains communication with the Senior Nurse Director, Staff and Medical Director or their designee for the purpose of coordinating operations between the department and the clinical division.
- KNOWLEDGE OF HEALTHCARE/BUSINESS SKILLS: Collaborates with nursing leadership (Nurse Director, Clinical Specialists and Nursing Education ) to ensure staff competencies. Participates in assessing staff performance against objectives; coaches, mentors, develops and evaluates employees to achieve behaviors aligned with the organization's vision, mission and values, and performance standards.
- KNOWLEDGE OF HEALTHCARE/BUSINESS SKILLS: Maintains continuing education files, competency assessment documentation, licensure and hospital certifications for self and staff.
- KNOWLEDGE OF HEALTHCARE/BUSINESS SKILLS: Works collaboratively with the Nurse Director to seek and utilize information (including research findings) to develop, implement and manage a budget which supports organizational objectives, patient care standards and quality of care. Demonstrates fiscal responsibility with day-to-day unit productivity and unit operations.
- KNOWLEDGE OF HEALTHCARE/BUSINESS SKILLS: Utilizes approaches and strategies that support and optimize patient care outcomes that are based on the interdisciplinary plan of care and absolutes of IMOC and Contemporary Primary Nursing.
- KNOWLEDGE OF HEALTHCARE/BUSINESS SKILLS: Takes action to support the National Patient Safety Goals, nursing sensitive indicators, regulatory requirements (e.g., DC Health, TJC) and other identified quality metrics.
- KNOWLEDGE OF HEALTHCARE/BUSINESS SKILLS: Supports and promotes quality and patient safety initiatives. Collaborates with physicians and associates to monitor and analyzes departmental performance data and identifies and participates in hospital and/or unit-based performance improvement activities including, but not limited to, core measures, nursing quality indicators, and other patient care outcomes (e.g., length of stay and patient satisfaction). Delivers direct patient care as required.
