JOB SUMMARY
1. Administers skilled nursing care to patients requiring intermittent nursing services according to the physician's orders and under the direction and supervision of the Director of Nurses.
2. Teaches and supervises patients and their families, caregivers, and significant others regarding issues relating to the patients health and well being.
3. Coordiantes all aspects of care for their assigned patients.
PERFORMANCE RESPONSIBILITIES
1. Complete holistic assessment including physical, emotional, social, and spiritual status.
2. Coordinates total patient care with all disciplines involved.
3. Carries out prescribed regimens and procedures and assesses outcomes.
4. Assesses patient and reports to DON, updates and changes in condition, efficiency of medications, and response to treatment.
5. Develops interventions aimed at assisting the patient to meet own needs.
6. Instruct, counsels and supervises patient and caregivers regarding patient care.
7. Participates in in-service education.
8. Attends in-service education.
9. Turns in daily visit records and nursing notes every twenty-four hours per Agency policy.
10. Reports to DON.
11. The LPN furnishes skilled nursing srvices by or under the supervision of a Registered Nurse in accordance with the plan of care.
12. Standard duties of a Licensed Practical Nurse.
13. Initiates appropriate preventive and rehabilitative nursing procedures.
14. Assists and participates in QA/QI activities.
15. Prepares clinical and progress notes, coordinates services, informs the DON and other personnel of changes in the patient's condition and needs.
16. Accepts on-call as scheduled.
17. Other duties as assigned by the DON or Supervisor.
18. Prepares clinical and Progress notes.
19. Assists the Physiian and RN in performing specialized procedures.
20. Prepares equipment and materials for treatment, observing aseptic techniques.
21. Assists patient in learning appropriate self-care techniques
