Social Worker 2 (MSW)
POSITION SUMMARY:
The Social Worker is responsible for providing social work services to all customer groups affected by illness/disability often including lifestyle adjustment difficulties. Understands utilization criteria and case management principles in a health care setting, as well as relevant community resources, insurance, regulatory, and legal system knowledge.
ESSENTIAL FUNCTION:
Are those tasks, duties, and responsibilities that comprise the means of accomplishing the job’s purpose and objectives. Essential functions are critical or fundamental to the performance of the job. They are the major functions for which the person in the job is held accountable. Following are the essential functions of the job.
1. Conduct Social Work Assessment/Education services:
a. Assesses, within the framework of a holistic approach, the social, emotional, familial and financial factors impacting the patient/family’s ability to cope with problems of daily living related to their mental health condition within, 48 hours of admission. b. Provide supportive education to individuals and families regarding such issues as:
i. Alternate care placement.
ii. Financial/insurance planning.
iii. Resources for D/C related needs.
iv. Preventative and protective services for patients and caregivers.
v. Crisis intervention.
c. Create treatment goals utilizing results of individualized assessments.
d. Coordinates inclusion input from patient support network in the therapeutic plan
e. Provide suicide risk assessments on psychiatric patients. Communicate findings to team, and recommend safety interventions based on findings.
f. Drafting of and setting parameters in behavioral plans.
g. Provide one on one therapeutic support.
h. Facilitates psycho-educational and other therapeutic groups.
i. May Assist with safety checks and other patient care-related functions including admission paperwork, searches, and other milieu management tasks as assigned
j. Assess acuity and levels of care
2. Provide Utilization Review Services:
a. Understand and utilize admission criteria and case management principles.
b. Liaison with area admission coordinators, RRMC case managers, staff, and physicians to facilitate alternate care placements.
c. Understand and abide by relevant legal and regulatory systems.
d. Conduct family and team meetings relative to care plans, including involved or potential outside providers.
e. May attend weekly organizational utilization review/ case management meeting
f. May complete monthly utilization review audits as assigned
g. May Obtain insurance authorizations for assigned patients
h. May complete admission, concurrent and discharge reviews with private insurance companies to ensure authorization of payment
i. May assist in prior authorizations of patient medications prior to discharge as needed after demonstrating competency.
3. Collaborate with inter-disciplinary team:
a. Advocate for patients within the organization.
b. Clearly communicates with the healthcare team and actively participates in comprehensive treatment planning.
4. Document clinical and professional activity:
a. Observes outcomes of interventions and records patient response in the patient’s record.
b. Provides clear, accurate and thorough assessments, treatment plans, and progress notes.
c. Construct patient profiles for alternative care placements.
d. Assist patients/families with referral forms for community services and resources.
e. Daily documentation of groups and services provided, patient participation and assessments of learning.
f. Process timely referrals for abuse/neglect reports and guardianship applications.
g. Complete required meeting logs, reports, statistics for any clinical projects, grants, support groups and/or administrative reports.
h. May participate in Utilization Review process.
i. Document all services and contacts provided in the electronic medical record.
j. Complete all documentation accurately in the electronic medical record in a timely manner, completed within the same day service is provided.
5. Provide information/referral services for community resources:
a. Maintains constant awareness of area resources, eligibility criteria, and updates on changes in these services.
b. Participates in maintaining area resource guide.
c. Keeps updated on out-of-area resource information relevant to patient need.
d. Advocates for system change to improve continuing care outcomes.
e. Collaborate with patient’s outpatient support system to cultivate beneficial advocacy with consideration to outpatient provider team.
6. Participates in required departmental activity:
a. Regularly attends required staff meetings, supervision, department meetings and related projects or committee meetings.
b. Completes work as assigned.
c. Participates in coverage of peers.
d. Demonstrates collegiality and commitment relevant to the department's mission.
e. Participates in maintaining database of group material and resources appropriate to RRMC patients.
f. May participate in On-call, after hours and /or weekend rotation (dependent on Department and role).
7. Provides social work consultation on request and coordinates closely with other area social and health agencies on patients’ behalf.
8. Attends all required safety training programs and can describe his or her responsibilities related to general safety, department/service safety, and specific job-related hazards.
9. Follows the home and community services division exposure control plans/blood borne and airborne pathogens.
10. Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
11. Promotes effective working relations and works effectively as part of a department/unit team interand in trade part mentally to facilitate the department’s/unit’s ability to meet its goals and objectives.
12. Demonstrates Commitment to Patient Safety
a. Understands and follows the organization’s patient safety-related policies, procedures and protocols.
b. Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes.
c. Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk.
d. Report potential or actual patient safety concerns in a timely manner per the organization’s protocol.
13. Acts as a Corporate Citizen:
a. Acts with integrity by adhering to policies and compliance requirements consistently and openly reports concerns to Leadership without fear of retaliation.
b. Is aware of the legal, regulatory and accreditation requirements that apply to their area.

14. Other duties as assigned.
15. Clinical Documentation:
a. Completes diagnostic and/or psychosocial assessments.
b. Conduct individual, group, and family therapy in accordance with identified treatment goals and assessment.
c. Provides suicide assessment screenings, identifies interventions, documents recommendations and communicates this to medical staff.
d. Provide screening for psychiatric consultations hospital-wide.
JOB REQUIREMENTS:
Minimum Education:
- Master’s degree in Social Work (MSW).
Minimum Work Experience:
- Experience in individual and family therapy.
- Five years’ experience working in healthcare setting preferred. Required Licenses/Certifications:
- Eligible for rostering in preparation for licensure.
Required Skills, Knowledge, and Abilities:
- Exhibits knowledge of Best Practice, DBT, CBT, and motivational interviewing.
- Provides employee assistance in-services, and consultation to staff, physicians, and community providers about psychosocial aspects of care.

Pay: $38,676.21 - $46,577.80 per year
