This is an outpatient model in a single building. No heavy lifting. You will make great connections with residents who will count on you to get them stronger, improve their mobility and enable them to feel better at the dusk of their beautiful lives.
Duties will include setting up schedules with clients, contributing your knowledge to supplement the goals for the residents. You will be asked to conduct a weekly exercise class for the community, if necessary. You will a welcomed and respected part of the community.

Responsibilities include:
· Following up on POCs and contributing your knowledge and expertise to the improvement of functional mobility of the clients.
· Assessing apartment environments for residents, making suggestions about adjustments to the layout for safety or suggestions of assistive devices.
· Conducting exercise regimens for the residents to regain balance and stability.
· Educating caregivers, if needed, in a plan of care and necessary tools for safety.
· Evaluating results and progress of the outcomes with timely charting on WebPT platform.
· Maintaining professional knowledge and monitoring progress in order to provide clients with the best treatment plan.
· Conducting an exercise class on a weekly basis for the community.
· Complying with federal, state and local licensing requirements, willingness to enhance professional knowledge of new treatments by participating in online seminars.
· Friendly collaboration with peers regarding schedules and care management.
Other requirements:
· PT and PTA licenses in California,
· Excellent interpersonal communication skills and team work.
· Ability to care for residents with different personalities.
· Enthusiasm and strong work ethics, and results – oriented attitude.
