Outreach Liaison

A TEAM OF DIFFERENCE MAKERS:
Author Health provides outpatient comprehensive behavioral healthcare and treatment which will improve health and general well-being through care coordination, connection to community-based resources, and technology facilitation for clinical visits. The outreach liaison role is responsible for increasing engagement of eligible patients through direct member outreach in the community, including in hospitals, sub-acute facilities, PCP offices, at member’s residences and telephonic outreach. This role will also spend time building relationships with community partners and educating them on the impact of our services to their eligible patients.
WHAT IS YOUR SUPERPOWER?
- Respond to Health Information Exchange (HIE) alerts (real-time notifications related to hospital admissions, transfers, or discharges) and meet with patients and their caregivers in the hospital or other care facilities
- Schedule intake appointments and where necessary facilitate on-demand visits
- In conjunction with local market leadership, build partnerships with local organizations, community leaders, and external providers to expand the reach of outreach and engagement efforts.
- Communicate and provide education on the value of Author Health to Health systems, PCP groups and other community related organizations
- Keep accurate records of all community interactions and patient outreach outcomes
- Identify and remove barriers to engagement with necessary resources and support
- Successfully meet and exceed monthly engagement targets
- Collaborate with Market Managers to identify partners and markets to focus outreach efforts on and support partner success team with account management
- Facilitate strategic initiatives with referral partners to drive engagement, i.e. embedded outreach and tandem visits in clinics
Your Role:
- Increase patient engagement through direct outreach in the community, including hospitals, clinics, homes, and by phone.
- Build relationships with community partners and educate them about our services.

- Schedule and facilitate patient intake appointments.
- Work with local leaders to expand outreach efforts.
- Keep accurate records of interactions and outcomes.
- Identify and remove barriers to patient engagement.
- Meet and exceed monthly engagement targets.
- Support partner success teams with account management and strategic initiatives.
What We Need:
- High school diploma/GED
- Strong organizational and communication skills
- Valid driver’s license and reliable transportation
- At least 1 year of relevant experience
- Time management and territory organization skills
- Passion for working with senior patients with behavioral health conditions
- Knowledge of community resources and healthcare delivery systems
- Willingness to travel up to 100 miles round trip per day
**Preferred:**
- Associate’s or Bachelor's degree in health/human services or a related field
- Bilingual (Spanish-English)
- Experience in direct-to-consumer sales or patient engagement in healthcare settings

Pay: $19.00 - $21.00 per hour
